Our Qualified Diabetes Educator is available to assist both inpatient and community clients needs.  Home Visits are available for legitimate request where it is not practical for a patient to attend the clinic and we have a Support Group for interested clients. The Diabetes Educator can assist people with the chronic disease triage of hypercholesterolaemia (high cholesterol), hypertension (high blood pressure) or hyperglycaemia (high blood sugars) better manage their lifestyle in order to reduce the complications of stroke, heart attack and diabetes. People with diabetes who have a General Practitioner Management Plan (GPMP) can access the new Medicare item numbers for Dietitican, Psychology and Optometry Services which will assist in effective self management of their diabetes. This service is available between the hours of 8.30am and 5.00pm on a Monday, Tuesday or Wednesday. Follow the links below to view some useful diabetes resources.
